Relevance of Palliative Care Training in Nursing Practice - Literature review Example Palliative care has generally been linked with end-of-life care of cancer patients, but considering its effectiveness in reducing the suffering of patients and their family the concept has now been extended to include non-cancer population as well. "Dying patients need to be assured that their destiny lies in the hands of well trained, multi-disciplinary, professional team because often the death is no longer capable of caring for themselves and their rights." (Gronemeyer, et al 2005, p.1). Hence, effective implementation of palliative care will be possible only through adequate training and grooming of health care providers. Since nurses interact most with patients and act as intermediaries between other health care providers and patients and their family members it is imperative that addressing the training needs of nurses is crucial for the success of palliative care program. As such, an attempt has been made to analyze select articles related to experimental models of palliative care and training strategies relevant to nursing practice. University of Wollongong Library's electronic resources was searched for locating relevant articles. Though Google scholar has larger volumes of articles, considering the fact that Proquest database covers specific nursing-related peer-reviewed journal articles, the search was limited to Proquest database only. The basic search question used was 'Palliative care', which retrieved 7982 articles from Proquest. A further search of the database for 'medical sciences-Nursing and allied health sources' shortlisted number of articles to 130. When the results were limited to full-text documents of scholarly journals 23 articles have been retrieved. Five articles with different area of coverage, such as (1) Nurses' perceptions around providing palliative care for long-term care residents with dementia; (2) Providing pediatric palliative care through a pediatric supportive care team; (3) Using high-fidelity simulation to educate nursing students about end-of-life care; (4) Enhancing meaning i n palliative care practice: A meaning-centred intervention to promote job satisfaction; and (5) Spiritual development of nursing students: Developing competence to provide spiritual care to patients at the end of life. Since large volumes of research articles (303,000) have been retrieved from Google Scholar using the catchphrase 'Palliative care' and only 5 articles have to be selected for the literature review it has been decided to search Proquest database and cover palliative care experiences in two challenging areas, such as pediatrics and geriatrics (dementia), and three articles covering innovative training approaches; Nurses' perceptions around providing palliative care for long-term care residents with dementia. The study conducted by Kaasalainen et al (2007) explores nurses' perceptions around providing palliative care for long-term care (LTC) residents with dementia, using a qualitative descriptive design. The inspiration for this study was the unprecedented increase in a ged care population with cognitive impairment and identification of 'many barriers to optimal palliative care in LTC homes' in Canada. My Most Beneficial Personal Quality

I have taken advantage of the education opportunities presented me thus far to prepare for college in many ways. First and foremost I have ensured that I have enjoyed a wide variety of classes and electives to provide me the opportunity to explore as broad a subject selection as possible. Most of the courses I have focused on in my senior year have been college preparatory. College preparation however is much more than academics alone. I have also experienced many educational opportunities that exist outside the realm of traditional ‘academics'. These experiences have prepared me more so for my college experiences than my preparatory courses. I have for example faced many challenges while growing up. During my four years of high school I engaged in a variety of extracurricular activities and clubs that taught me much more about the ways that people interact and communicate than I ever would have initially assumed. Participating in many volunteer organizations and student achievement groups, I realized that not two people interact in the same way. Most people I learned are products of many different elements including their beliefs, values, morals and faith. One's religious ideals, culture and even familial upbringing can all influence their perceptions on life, their ability and motivation to interact with others and their desire to achieve. With this knowledge I learned new and exciting ways to interact with people, even those that were very different from me and with those that held very different beliefs. In fact in most of my pursuits I found myself drawn to those with conflicting ideals and moralisms, in part because they challenges me to learn more about myself and question the beliefs I held true about the world in which we live. I believe it is through these types of interactions that we actually learn everything there is to know of the world. We need to rely on people that challenge us and our views so we open our eyes and get a better understanding of the world in which we live. I feel these diverse educational experiences while not academic, have prepared me for life in the university more so than any classroom instruction may have or will in the future. I bring to the University of California a unique ability to network and work personally with people. By far my most beneficial personal quality is my ability to build relationships and communicate with a wide diversity of people from varying backgrounds, cultures and environments. In today's society it is vital for anyone to understand the importance of interpersonal skills and relationships both on a personal and from a professional perspective. More and more the world is operating in a global business environment. Modern technology has afforded people the opportunity to communicate from once remote locations. What this means is that people from all over the world with all manner of different morals, values and beliefs are not just communicating with one another but also working together to create a better world. As members of a global society I feel it is vital that we all have the ability to communicate effectively with one another. Sometimes this takes the guidance and direction of a true interpersonal leader. I believe I am possessed of those traits. I have throughout my high school career taken great care to interact with people from various backgrounds and ethnicities. I have participated in multiple groups, clubs and organizations that have enabled better exposure to different people and have helped me learn better ways to communicate with one another. I have also witnessed first hand what happens when communication goes bad, and have since committed myself to facilitating solid communications and interactions between people. I believe this personal trait will benefit the University of California in many ways. The University of California is one of the nations top universities; home to thousands of unique students and home of many of the nations top academic programs. Early on I knew that more than anything I wanted to pursue the best education I could afford. To succeed in today's world business professionals and students must be possessed of a wide range of skills that are best learned in a University environment. I feel that throughout my life I have already faced many unique challenges and obstacles. I was not always a committed student, lacking the insight and motivation necessary to excel in my classes early on. However experience has taught me that little in the world is more valuable than an education. Every day on the news we hear stories of tragedy. Hurricanes that devastate infamous regions, terrorism that destroys the livelihood and communities of varying regions†¦ these are all modern realities we must learn to cope with. To address the often-turbulent environment we live in today, it is vital that we all commit ourselves to one another and our communities. I feel that I can best do this by excelling in my education. Through my university education I hope to learn not only of the fine nuances of other communities, but also about key demographics about the worlds' population and needs entering the upcoming decades.

It feels strange running into something that everyone else is running out of, it’s my second time today. I can’t explain the thrill of running into the blaze, for some it’s just a career but for me it’s my passion. As I reached the top of the burning stairs I looked out the window to see Don with his back against the truck again, he was just standing there watching whilst men are in that house risking their lives to save others. It angers me so much to know that his name will be on the front page tomorrow for what I’ve done again. When we got back to the station the others applauded Don as he walked in and exclaimed â€Å"another fire stopped and 3 lives saved† me and Benji couldn’t bear to watch him take the glory for what we had done, so we went outside to take a breather and collect the post. Whilst we were out there we met the postman, he was standing there as if he was waiting for us, me and Benji exchanged a confused glance and then I said hello, he muttered â€Å"you’ll be station manager son† â€Å"sorry? † I said â€Å"and station commander† he replied â€Å"it’s as if I can tell the future! he laughed as he turned to walk away â€Å"what about me then eh? † shouted Benji shouted after the postman â€Å"it’ll be your sons and his sons and his sons and his sons† the postman sang as he skipped away. Benji and I couldn’t help but burst into laughter, at least it had lightened our mood after Don’s little display inside. That night I told Beth all about what Don has been doing and how he has been taking all the credit lately, I just wanted to vent to her but she got so much angrier than I had expected and told me that I should get rid of him by killing him! I want him gone and I want the praise but is this too extreme? She went on to tell me that if I didn’t do it I’d be a coward and there’s no point in me going back to the fire station at all, I can’t imagine not going back into a fire ever again. I’ve realised I have to do this. I told Beth I agree and she had a plan already. I’m going to kill him in the old house on foster street on Tuesday night when we go there for a risk assessment, Beth told me it would be perfect and no one will know I had anything to do with it; I’ll set the place alight and blame it on the squatters there.

The word, cult, has a negative connotation which reflects the rejection of the populace in which it exists. The word cult is often used to express disapproval of a scorned religion different from one's own. A cult is defined as; "...a system of religious worship..." or "...a creed or sect..."(The New Lexicon Webster's Dictionary of the English Language) While the dictionary definition of religion is, "...man's expression of his acknowledgment of the divine..." or "...a system of beliefs and practices relating to the sacred and uniting its adherents in a community..." (The New Lexicon Webster's Dictionary of the English Language) The two words are synonymous by definition, but the ways in which they are used contrast because of their differing connotations. Once a group is labeled a cult it is subjected to some form of prosecution. One group will attack another, either because they simply feel they are right and the other group's belief system must therefore be wrong. The group becomes an outlier and is banished, censured, or ostrasized. When labeled a cult like the branch Davidians of Waco, Texas the group is subject to infiltration and forced collapse. The Freedmen's Bureau - Agency to Assist Former Slaves The Freedmens Bureau was created by the U.S. Congress near the end of the Civil War as an agency to deal with the enormous humanitarian crisis brought about by the war. Throughout the South, where most of the fighting had taken place, cities and towns were devastated. The economic system was virtually nonexistent, railroads had been destroyed, and farms had been neglected or destroyed. And four million recently freed slaves were faced with new realities of life. On March 3, 1865, the Congress created the Bureau of Refugees, Freedmen, and Abandoned Lands. Commonly known as the Freedmens Bureau, its original charter was for one year, though it was reorganized within the war department in July 1866. The Goals of the Freedmens Bureau The Freedmens Bureau was envisioned as an agency wielding enormous power over the South. An editorial in the New York Times published on February 9, 1865, when the original bill for the creation of the bureau was being introduced in Congress, said the proposed agency would be: ... a separate department, responsible alone to the President, and supported by military power from him, to take charge of the abandoned and forfeited lands of the rebels, settle them with freedmen, guard the interests of these latter, aid in adjusting wages, in enforcing contracts, and in protecting these unfortunate people from injustice, and securing them their liberty. The task before such an agency would be immense. The four million newly freed blacks in the South were mostly uneducated and illiterate (as a result of laws regulating slavery), and a major focus of the Freedmens Bureau would be setting up schools to educate former slaves. An emergency system of feeding the population was also an immediate problem, and food rations would be distributed to the starving. It has been estimated that the Freedmens Bureau distributed 21 million food rations, with five million being given to white southerners. The program of redistributing land, which was an original goal for the Freedmens Bureau was thwarted by presidential orders. The promise of Forty Acres and a Mule, which many freedmen believed they would receive from the U.S. government, went unfulfilled. General Oliver Otis Howard Was Commissioner of the Freedmens Bureau The man chose to head the Freemens Bureau, Union General Oliver Otis Howard, was a graduate of Bowdoin College in Maine as well as the U.S. Military Academy at West Point. Howard had served throughout the Civil War, and lost his right arm in combat at the Battle of Fair Oaks, in Virginia, in 1862. While serving under Gen. Sherman during the famous March to the Sea in late 1864, Gen. Howard witnessed the many thousands of former slaves who followed Shermans troops on the advance through Georgia. Knowing of his concern for the freed slaves, President Lincoln had chosen him to be the first commissioner of the Freedmens Bureau (though Lincoln was assassinated before the job was officially offered). General Howard, who was 34 years old when he accepted the position at the Freedmens Bureau, got to work in the summer of 1865. He quickly organized the Freedmens Bureau into geographical divisions to oversee the various states. A U.S. Army officer of high rank was usually placed in charge of each division, and Howard was able to request personnel from the Army as needed. In that respect the Freedmens Bureau was a powerful entity, as its actions could be enforced by the U.S. Army, which still had a considerable presence in the South. The Freedmens Bureau Was Essentially the Government in the Defeated Confederacy When the Freedmens Bureau began operations, Howard and his officers had to essentially set up a new government in the states that had made up the Confederacy. At the time, there were no courts and virtually no law. With the backing of the U.S. Army, the Freedmens Bureau was generally successful in establishing order. However, in the late 1860s there were eruptions of lawlessness, with organized gangs, including the Ku Klux Klan, attacking blacks and whites affiliated with the Freedmens Bureau. In Gen. Howards autobiography, which he published in 1908, he devoted a chapter to the struggle against the Ku Klux Klan. Land Redistribution Did Not Happen As Intended One area in which the Freedmens Bureau did not live up to its mandate was in the area of distributing land to former slaves. Despite the rumors that families of freedmen would receive forty acres of land to farm, the lands which would have been distributed were instead returned to those who had owned the land before the Civil War by order of President Andrew Johnson. In Gen. Howards autobiography he described how he personally attended a meeting in Georgia in late 1865 at which he had to inform former slaves who had been settled onto farms that the land was being taken away from them. The failure to set former slaves up on their own farms condemned many of them to lives as impoverished sharecroppers. The Educational Programs of the Freedmens Bureau Were a Success A major focus of the Freedmens Bureau was the education of former slaves, and in that area it was generally considered a success. As many slaves had been forbidden to learn to read and write, there was a widespread need for literacy education. A number of charitable organizations set up schools, and the Freedmens Bureau even arranged for textbooks to be published. Despite incidents in which teachers were attacked and schools burned in the South, hundreds of schools were opened in the late 1860s and early 1870s. General Howard had a great interest in education, and in the late 1860s he helped to found Howard University in Washington, D.C., a historically black college which was named in his honor. Legacy of the Freedmens Bureau Most of the work of the Freedmens Bureau ended in 1869, except for its educational work, which continued until 1872. During its existence, the Freedmens Bureau was criticized for being an enforcement arm of the Radical Republicans in Congress. Virulent critics in the South condemned it constantly. And employees of the Freedmens Bureau were at times physically attacked and even murdered. Despite the criticism, the work the Freedmens Bureau accomplished, especially in its educational endeavors, was necessary, especially considering the dire situation of the South at the end of the war.
